The athletic/activities department asks that all parents and all students view and complete the requested forms PRIOR to the student attending practices.
Three (3) forms are required to be on file in order to participate in sports/activities:

MSHSL Athletic Eligibility Form (Form needs to be completed annually - by both parents & students - Grades 7-12). Hard copy document below.
Physical Exam Form Required every three years, signed & dated by physician, Grades 7-12. Require for participation in any Minnesota State High School League (MSHSL) sponsored sport or activity. Please see MSHSL website (www.mshsl.org) for complete list of sports/activities. We accept new physicals all year long -- feel free to fax (952-926-5764), scan, email (jkoski@bsmschool.org) or drop off at athletic office at any time. Hard copy of MSHSL physical form is below.
Parental Form for Alternate Transportation (for those occasions when student will NOT be taking transportation provided (i.e. bus) NOR will student be riding home with parents -- please complete and turn into coach)
Transcript Request for College Athletic Departments - contact your student's counselor.
Team Photos -- BSM uses LifeTouch Sports for the majority of team/indiviuals photographs. BSM recommends that ALL individuals get an individual photo taken during the team picture day process. This avoids some confusion for the individual student. Ordering photos is available by the following procedures.
